Job Description:
The provider will be responsible for coordinating care and providing therapeutic support to children and families. Responsibilities include, but are not limited to, the following:
1. Case Management:
Client Intake and Assessment:
- Conduct intake interviews to gather personal, medical, social, and psychological histories.
- Perform needs assessments to identify the client’s challenges and resources required for support.
Develop and Implement Care Plans:
- Create individualized service plans based on clients’ needs and goals, outlining specific interventions and resources.
- Collaborate with clients, families, and multidisciplinary teams to ensure comprehensive care.
Coordinate and Monitor Services:
- Connect clients to relevant services, such as housing, healthcare, education, employment, and mental health services.
- Monitor progress, ensuring that clients receive timely and appropriate support.
- Advocate for clients by liaising with external service providers and agencies (e.g., legal, healthcare, social services).
Documentation and Reporting:
- Maintain accurate case files, including assessments, plans, progress notes, and any required reports.
- Regularly update client records and ensure compliance with legal and organizational requirements.
Crisis Intervention and Problem Solving:
- Provide immediate support and interventions during crises, such as mental health emergencies, homelessness, or domestic violence situations.
- Develop and implement safety plans when necessary.
2. Counseling Services:
Provide Individual and Group Counseling:
- Offer therapeutic counseling sessions to help clients address emotional, psychological, and behavioral challenges.
- Use evidence-based therapeutic approaches (e.g., Cognitive Behavioral Therapy, Solution-Focused Therapy) tailored to the client’s needs.
Support Emotional and Mental Health:
- Help clients process trauma, manage stress, and cope with mental health issues such as anxiety, depression, or addiction.
- Provide education on coping strategies, stress management, and resilience building.
Assist in Goal Setting and Personal Growth:
- Work with clients to set personal goals related to their emotional well-being, family dynamics, education, or career.
- Empower clients to develop self-management skills and increase independence.
Address Co-Occurring Disorders:
- Support clients dealing with multiple challenges, such as mental health issues and substance abuse, by coordinating care across various treatment modalities.
Family and Relationship Counseling:
- Facilitate family counseling sessions to address family dynamics, improve communication, and resolve conflicts.
- Work with couples or families to strengthen relationships and support positive change.
3. Collaborative Work and Advocacy:
Multidisciplinary Team Collaboration:
- Partner with other professionals (e.g., doctors, social workers, educators, law enforcement) to ensure holistic care.
- Participate in team meetings, case conferences, and treatment planning.
Advocacy:
- Advocate for clients’ needs within healthcare, education, and legal systems.
- Educate clients on their rights and help them navigate complex systems.
4. Follow-Up and Continued Support:
Ongoing Case Review:
- Regularly review and adjust care plans to meet changing client needs.
- Provide continuous support and check-ins to monitor progress and address emerging challenges.
Discharge Planning:
- Assist in planning for clients’ transitions out of services when goals are met, ensuring sustainable supports are in place.
Qualifications:
- Master’s or Doctorate degree in social work, counseling, psychology, mental health, or a related field (e.g., certified psychology assistant, LADAC Level I or II).
- Licensed in the state of Tennessee or pursuing licensure.
- Familiarity with trauma-informed care, mental health treatment, and substance abuse interventions.
- Previous experience in a clinical setting.
